New Member Enrollment Forms
To be eligible to join the ARA, you have to be a member of the Shia Imami Ismaili Community. If you are eligible, please submit the following to the ARA office:
• Application Form - completed
• Survey Form - completed
• Copy of primary member’s driver's
• Copy of business license
• Copy of articles of incorporation
• $175.00 check or money order payable to the ARA
Note: Your Account will not be opened until all the above mentioned documents are received by the ARA office.

Vendor Forms
Information for potential vendors
If you are interested in becoming an ARA endorsed vendor, please read the following:
• Submit a vendor application along with a proposal via mail providing your company profile, including product specifications, rates, ARA Benefits, discounts to ARA Members, references, current clients/customers, contact information and any other relevant information.
• Once your proposal is received, it will be reviewed by our Marketing & Promotions Team.
• If approved the ARA will get in touch with your company to initiate discussions. Assuming a mutually beneficial agreement is reached, a contract will be executed to that effect with your company.
